The Overview

Attention is where you point your mind. Focus is your ability to keep your attention steady while other things pull at you.

If you want to understand your emotions, step one is to place your attention on them. Step two is to build the skill to stay focused on them. Emotions are just information. They are physical signals in your body telling you something is happening and pushing you to act—like warning lights on a car dashboard.

But a dashboard only helps if you actually look at it. If you cannot hold your focus on your own body, you won’t feel the heat rising in your chest. The first time you realize you’re angry will be when you hear yourself yelling. By then, you are already reacting. Pointing your attention inward buys you a few seconds in between. It gives you a gap. Everything else we cover in this course runs on that gap.

Maintaining that focus is difficult because our attention is constantly being pulled somewhere else. We spend about half our waking hours thinking about something other than what we’re actually doing. And no kind of daydreaming beats simply being absorbed in what you’re doing—pleasant daydreams leave us no happier, and the rest leave us measurably worse (Killingsworth and Gilbert, 2010).

You can train this, and the way it is usually trained is through attention practice—what most people call mindfulness or meditation. But the way most people understand meditation sets them up to fail. They try to sit still, their focus immediately breaks, and they quit because they think wandering means they are bad at it.

Attention wanders. That isn’t a problem you can fix. The wandering is the exercise. The skill is the return: noticing your attention drifted, and bringing your focus back without beating yourself up. Every time you drift and come back, you just did one repetition.

Think about shooting a basketball. The best players in the world miss about half their shots, and they never stop missing. Nobody watches a pro miss a shot and decides they don’t know how to play.

Focus works exactly like that. When scientists put experienced meditators—people with over a thousand hours of practice—into a brain scanner and asked them to watch their breath for twenty minutes, they still lost focus about fifteen times. That’s roughly once every eighty seconds (Hasenkamp and colleagues, 2012). These were experts, and their attention still wandered.

You won’t make every shot. But you can make a lot more than you make now. Practice makes your attention drift less often (Mrazek and colleagues, 2013). That adds up—fewer trips away over the course of a day. You don’t get a perfectly silent mind; you just get to spend far more of your day exactly where you meant to be.

It also takes a lot less time than you might think. Just thirteen minutes a day changed attention, memory, and mood in people who had never meditated before—at eight weeks, though not yet at four (Basso and colleagues, 2019). Aim for at least ten minutes a day, every day. One minute a day is a completely fine place to start. A session can even be seconds long. Yongey Mingyur Rinpoche, world-renowned Tibetan Buddhist meditation master, teacher, and bestselling author, advises short times, many times—take one intentional breath, many times a day, while you wait for the kettle to boil or before you open your laptop. Short, frequent practice works, not just longer sessions (Fincham and colleagues, 2023; Cearns and Clark, 2023), and a tiny practice you actually keep beats a serious one you quit by day three.

This kind of practice makes you notice more of what you feel. Noticing and allowing work together: attention finds the feeling, and allowing it to be there is what turns noticing into something useful. On its own, monitoring what you feel doesn’t move the stress response (Lindsay and colleagues, 2018). For some of us, turning the volume up on our internal world brings out more than we can handle on our own right now—things like deep grief, fear, or a sense of watching your life from the outside.

If tuning into your body triggers that kind of overwhelm, you don’t have to push through it, and you don’t have to quit. Just change where you point your spotlight. Use an external anchor instead—stare at a candle flame, a spot on the wall, or a blade of grass. You can build your focus without having to wade through a flood of unresolved emotion.

The Challenge

In one of the most telling studies I’ve seen, researchers put people in a brain scanner and placed a small heat pad against their forearm. At first, they turn the pad on and it’s just mild and comfortable. Twelve seconds later the heat jumps — half the time to a temperature that is actively painful, half the time to one that isn’t. Then they let it cool, wait, and warm the pad up again. They keep repeating this loop.

The participants quickly catch on: when they feel that mild heat, pain is possibly twelve seconds away. They start anticipating it, and the same brain regions that handle the pain start working before it arrives.

They begin to get nervous the second the mild heat shows up. Round after round, the beginners brace more heavily than the round before. A temperature that cannot hurt anyone is now driving the threat machinery (Lutz and colleagues, 2013).

Human life works a lot like that experiment. Painful things happen, and they hurt. There isn’t much we can do about that. But our brains struggle to accept that. We spend a massive chunk of our lives bracing for what might be coming. Some of that is helpful — planning can help us dodge unnecessary pain.

The rest of it is the warm patch. We drag future pain (anxiety) or past pain (rumination) into the present moment, playing it on a loop in a body that is currently perfectly safe. Because our imaginations are so powerful, much of what we rehearse never actually happens. When researchers asked chronic worriers to write down their fears and check back later, about 91% of the things they worried about never came true (LaFreniere and Newman, 2020). Nearly all of it was suffered anyway. And the backward-looking version carries its own bill: people who ruminate heavily are far more likely to slide into depression (Nolen-Hoeksema, 2000).

Meanwhile, the present moment gets hollowed out. When researchers checked in with thousands of adults at random moments throughout the day, they found people were thinking about something other than what they were doing about 47% of the time. The result was clear: no version of drifting beat simply being present (Killingsworth and Gilbert, 2010).

Put that together. When we lack the ability to steer our own attention, we do two things at once: we pull fake pain from the past and future into the present, and we water down whatever is actually in front of us.

When you move through life completely unaware of how your focus is drifting, your attention becomes a free-floating resource. And if you aren’t directing it, someone else will happily move it for you.

The modern world is engineered to capture that drifting attention. The entire field of marketing is designed to find a feeling you already carry — that you are behind, that you aren’t enough, that you are unsafe — and hang a product on it. Our movies, feeds, news cycles, and cultural pressures are all built to tap into your emotional world and push you toward a specific action.

In a global economy that sells consumption as the primary route to feeling better, this hijacking of our attention gets disastrously expensive. Across hundreds of samples, the people who put the most weight on money and possessions report lower wellbeing, not higher (Dittmar and colleagues, 2014). We buy the product, but the underlying emotional need stays untouched. So we buy more. We are largely asleep at the wheel while we wreck the planet, warp our body image, lash out at the people near us, build addictions, and slide into depression.

It doesn’t stop there. Bodies aren’t built to run on constant systemic stress. When the alarm never switches off and the culture keeps pushing, something has to give — so we start turning the signal down. Feelings get quieter, or stop arriving. When the overwhelm gets large enough, people leave their bodies altogether — a sense of watching your own life from somewhere outside it, or going blank and losing stretches of time, called dissociation.

That reads like a list of faults, but it isn’t one. Every item on that list worked. Numbing works. Going flat and saying you are fine works. If being fully present meant being overwhelmed, or terrified, or in pain, then checking out may have been the best choice available to you. Your mind was incredibly smart to learn it. None of this is a flaw in your character; it is a survival strategy.

For some of us, that strategy is still running long after the actual threat has passed. When a brain gets wired for danger early on, the alarm system gets stuck in the “on” position, even if you are lucky enough to have finally made it to a safe place. For others, the danger hasn’t passed. You might still be in an environment that is genuinely unsafe, or a system that is actively hostile, and you are simply looking for a way to find a few minutes of calm between the shocks.

Whatever your reality is, if this lands close to home — if the habit of leaving your body is old, deep, and familiar — there is no rush here. Forcing yourself to suddenly strip off the armor and “stay present in your body” may not be what you need. It will only make the alarms ring louder. Shifting your attention and focus toward an anchor outside of your body can be a gentler start. If you recognize these patterns in yourself, and you decide you would like to try living a different way, take it slowly. When you feel overwhelmed, pause there. Otherwise you are likely to stop the practice, or begin leaving your body.

The trouble with turning the signal down is what gets lost along with the pain. Emotions aren’t good or bad. They are information — lights on a dashboard telling you what is happening under the hood, and pushing you to act. Turning them down doesn’t fix the car. It just means nobody is reading the warning messages. Listening to signals takes attention: just enough to stay pointed at your own chest for a few seconds while it reports. Without that, the lights keep flashing for threats that aren’t there, and the messages that actually matter go unread.

The Benefit

Another group sat in that same scanner.

They got the exact same heat, the same warm patch, and the same warning. But these people had been training their attention and focus for a very long time. Every one of them had at least ten thousand hours of meditation practice behind them, and most had far more.

They didn’t feel less heat. When asked to rate how hot the pad was, they gave the exact same answers as everyone else. It bothered them less (Lutz and colleagues, 2013).

Underneath that answer, their brains were doing something entirely different. During the warm patch, before anything hurt, their pain regions and their amygdala — the brain’s threat-detection center — were significantly quieter than the beginners’. The more hours a person had practiced across their life, the quieter those regions ran. Then, during the actual heat, the network that decides what is worth your attention worked harder in the experts than in the beginners. But across the rounds, the experts settled while the beginners wound tighter. They were less braced beforehand, more present during, and faster to recover afterward.

The heat still hurt. Ten thousand hours did not make the heat any less hot, and no amount of practice takes pain out of a human life. What changed was what they were adding to it: the dread in front of it, the resistance during it, and how much each round loaded the next one.

A 2,500-year-old Buddhist teaching explains this perfectly. Imagine you are suddenly shot by an arrow. It hurts terribly. That is the unavoidable pain of life — the physical injury, the sudden loss, the harsh words. But what do most of us do next? We thrash around, we panic, we curse the person who shot us, we stress about whether it will ever heal, and we agonize over why this always seems to happen to us.

In doing so, the teaching says, we shoot ourselves with a second arrow.

The first arrow is the actual event. We rarely have control over it. The second arrow is the mental suffering we add to it. The untrained person feels two pains: physical and mental. The trained person feels the first arrow, but they do not fire the second (Sallatha Sutta, translated by Thanissaro Bhikkhu).

The brain scanner essentially measured those two arrows. “How hot was it?” is the first arrow, and the experts’ answers matched everyone else’s. “How much did it bother you?” is the second arrow, and that is the number that came down. The dread the beginners felt during the warm patch was just the mental arrow being fired twelve seconds early.

The offer here isn’t a perfectly silent mind or never firing the second arrow again. These were experts with decades of practice, and they still found the heat unpleasant. You fire that second arrow less often.

This isn’t just one striking study. It replicates across the field: pooling 47 randomized trials and 3,515 people against real comparison groups, mindfulness training produced reductions in anxiety, depression, and pain in the same range antidepressants and CBT do (Goyal and colleagues, 2014).

Almost everything painful about an ordinary week is second-arrow material. The hour spent sweating over a conversation that ends up going fine. The worries about the future that never actually happen. The argument replayed at two in the morning in a house where nothing is actually wrong. None of that is the first arrow. All of it is what we add.

This doesn’t mean you stop thinking about the past or the future. Thinking about what is coming is how you prepare, and thinking about what happened is how you learn. The difference is in how you do it. Concrete thinking — what do I actually say to her on Tuesday? — tends to help. Abstract, spiraling self-judgment — why do I always ruin things? — does not have an answer. Asking it isn’t working on a problem; it’s just firing more arrows (Watkins, 2008).

The goal of attention practice isn’t an empty head; it is a mind you can actually steer — the ability to realize you are thinking, and to ask yourself: Am I working on this, or am I just spinning?

From the inside, it doesn’t feel like perfect concentration; it feels like a shorter leash. Your mind still wanders — it just wanders less often. Before training, your mind leaves a difficult conversation and by the time you catch it, you cannot remember the last few pages that you read, and have to work to find your place again. With training, you can spend far less time wandering and backtracking. You stay more present and aware of each word as you read it.

That shorter leash changes how a normal day feels. A drifting mind gets captured by algorithms, feeds, and highly engineered distractions, and when you are used to that level of constant, frantic stimulation, quiet moments start to feel like an uncomfortable itch.

When you learn to anchor your attention, those quiet things come back. Sitting on the couch with nobody talking, or playing on the floor with an infant, stops reading as itchy boredom because you can finally hold your attention there long enough to enjoy it. And when you know where your attention is, the endless pulls of modern life start arriving as recognizable tugs rather than as your own ideas. A tug you can notice is a tug you can decline.

Most importantly, turning up your ability to notice what is happening inside you, without the separate skill of allowing what you find, is not what does the work — and for some people it surfaces more than they can hold. If your emotions went quiet years ago for a good survival reason, turning the dashboard back on is a delicate process.

But turning that dashboard back on is the entire foundation of this work.

Without focus, we cannot see our thoughts. We cannot accurately notice our feelings while they are still quiet enough to work with. If we can’t see the emotion, we can’t examine the beliefs beneath it, explore the actions it points us toward, or understand how those actions impact the people around us.

Attention creates a gap between what you feel and what you do. Everything else is about what you do inside that pause.

The Practice

The wiring that lets us animals put our attention on one thing out of everything else is at least 450 million years old (Knudsen, 2018; Isa and colleagues, 2021). The same system still runs in everything from lampreys to us.

Attention is a skill, and it is built like any other capacity: regular practice and training.

There are as many ways to train your attention as there are places to put it, which makes the list practically limitless. That can feel overwhelming when you are staring at a page of exercises, but they all come down to the same repetition: you hold your attention on something — an anchor — and when you drift, you come back to it. That is the same event no matter what you chose.

You can pick an anchor in your internal world. That might be the physical sensation of your breath, the tip of your nose, or a slow scan of your body. You might focus on the crown of your head, the points along your spine, your toes, or even your thoughts themselves.

But for some people, internal anchors just don’t feel right. If focusing inward is uncomfortable or distracting, no big deal—start with something else. Give your attention to the external world. You can stare at the flame of a candle, a single spot on the wall, or the entire wall. You can watch a bug crawling on the sidewalk or a single blade of grass. The “right” anchor is simply the one you are willing to use.

Try a few before you settle, because our guess about which anchor will suit us is usually not very good. When researchers gave 82 people who had never meditated three anchors in a row—the breath, a repeated phrase, and a simple image on a screen—about half ended up preferring the breath and the rest split between the other two. More than half of them ended up preferring a different anchor than the one they had picked before trying (Anderson and Farb, 2018).

Personally, my favorite anchor to start a session with is a single external dot, or something equally small. I like looking at a tiny object because I notice right away when my mind drifts — my eyes lose focus and the object blurs, so I know instantly that my attention wandered. After about five or ten minutes of that, my mind is calm enough to move to something else. Some days that is my breath or the sensations moving through me; other days something outside, like a sound or a candle flame or a person talking to me. Sometimes it is an idea I want to sit with, or a phrase I repeat. And sometimes it is nothing fixed at all — just an open awareness of whatever arrives.

You just have to find what works for you. The only wrong way to do this is not paying attention.

Attention doesn’t always have to sit deep on one thing. Sometimes the better move is to zoom out — a broad, open awareness that isn’t locked onto anything in particular. It sounds like the opposite of training: how do you get better at not focusing on something? Open awareness is a real, specific skill. Training it means catching more of what is actually in front of you, including things that pass too fast for most people to see at all (Slagter and colleagues, 2007). Elite athletes whose sport is reacting to whatever comes at them score better on that kind of open, flexible attention than elite athletes whose sport is the flawless execution of a pre-planned routine (Koch and Krenn, 2021).

Depth on one point and breadth across everything are both trainable. They are not the same skill. Develop your attention and focus skills where you will need them.

Those choices are not interchangeable. Across 78 brain-imaging studies, the major styles of practice ran on largely separate machinery, and in each case the machinery matched the job (Fox and colleagues, 2016). You get the one you train. Shown an image that normally flips back and forth on its own, Tibetan monks doing one-point concentration slowed the flipping down; for three of them — two of the most experienced retreatists in the group — it stopped completely for the full five minutes. The effect disappeared the moment they switched to compassion meditation (Carter and colleagues, 2005). Meditators trained in open awareness beat meditators trained in concentration when a signal arrived unexpectedly, but not when it arrived on schedule (Valentine and Sweet, 1999).

Early on this matters less. You are building the basic ability to hold your attention and bring it back, and almost any anchor does that. The further in you go, the more what you choose decides what you get.

Catching your mind wandering and pulling it back to your anchor is the practice — the repetition that builds the muscle. Each of those four moments—attention on the anchor, the mind leaving, the catch, the return—shows up as its own distinct activity in the brain (Hasenkamp and colleagues, 2012). This is the most important part of the entire process: the moment you catch yourself not paying attention, you are already back. You haven’t failed.

One exception is worth naming. Sitting still with your own mind takes away the distraction that was covering whatever is underneath it. For most people what is underneath is boredom and a to-do list. For some—especially anyone carrying something that has never been dealt with—it is grief or fear arriving with nothing in the way. Watching the breath has its own version of this: for some people it makes breathing feel effortful and strange, and the strangeness turns into panic.

Neither of those means you are doing it wrong, and neither one means push harder. Make the sessions shorter. Keep your eyes open. Use an anchor outside your body—sound, a dot on the wall, your feet on the floor—instead of one inside it. And get another person involved rather than grinding through it alone. If the habit of leaving your body is old, deep, and familiar, start with an external anchor and come back to this when you are ready.

Start small, keep track, and slowly increase the challenge. Again, this is no different than learning a sport or an instrument. If you try to sit still in silence for an hour on your very first day, it will likely be too difficult and you will stop.

Instead, try one minute a day for the first week. Just one minute, but do it every single day. The next week, go up to two minutes. Keep stepping it up week by week.

Build to ten minutes a day, and do it every day.

Ten is not a number someone guessed at. When researchers put ten-minute and thirty-minute daily sessions head to head, the two groups came out the same on well-being, distress, and mindfulness (Fincham and colleagues, 2023). Going longer is fine. It is just not where most of the benefit comes from.

For myself, my sweet spot is about 20 to 40 minutes, twice a day, which is more than most people need. Yours might be different.

Small daily amounts really do work, and they take a while. In one study, people who had never meditated were randomly assigned to either thirteen minutes a day of guided meditation or thirteen minutes a day of listening to a podcast, for eight weeks. Attention, memory and mood all improved in the meditation group—at eight weeks, but not yet at four (Basso and colleagues, 2019). If you are five weeks in and nothing has changed, that is roughly what the research would predict.

In one way this is not like learning an instrument. With an instrument, effort is the main lever. With attention, sleep is. Simple attention is less a skill you build than a state your body has to be in, and the longer you are awake the harder the pressure to sleep pushes back — until the brain starts taking small amounts without asking, in lapses you do not choose and often do not notice. You cannot decide your way out of that. Across about seventy studies of short-term sleep loss, lapses of simple attention took the biggest hit of anything measured, while the ability to reason barely moved (Lim and Dinges, 2010). If you are sleeping five hours a night and practicing ten minutes a day, the ten minutes is not what is holding you back. Fix the sleep first.

If you want a little more structure, there are several excellent apps and programs available, many of them free. Most include guided sessions where you practice focusing your attention on specific ideas—like compassion, forgiveness, impermanence, or gratitude.

Jon Kabat-Zinn took the plain practice of anchoring your attention and bringing it back, built it into an eight-week course for hospital patients, and called it Mindfulness-Based Stress Reduction — MBSR.

MBSR taught live over video works about as well as MBSR taught in a room (Hoge and colleagues, 2025), and the app studies report effects in the same general range as the in-person trials. Pick the one you will keep opening. There are a few exceptions worth knowing. Social anxiety improved more when people were physically in the room together, which makes sense, since sitting with strangers is the thing being practiced. And among online programs, having a real person supporting you roughly doubled the benefit for stress, while making no difference to depression, anxiety, or well-being (Sommers-Spijkerman and colleagues, 2021). If you are dealing with a lot of stress or depression, talking it through with a professional tends to help.

Free and ad-free; you do have to register.

Palouse Mindfulness — the complete eight-week MBSR course, free, with no account, no signup, and no payment of any kind.

Insight Timer — the largest free library of guided practice anywhere. It does require an account, and most of the structured courses sit behind a paid tier.

Those three you work through on your own schedule. These two are taught, with a live instructor and a group:

MBSR at the UMass Center for Mindfulness — the original eight-week course, taught live in a cohort over video: weekly two-and-a-half-hour classes, a full-day session, and one-on-one time with an instructor. It costs $650.

MBCT at the UMass Center for Mindfulness — an eight-week course in Mindfulness-Based Cognitive Therapy, or MBCT: the MBSR structure with cognitive therapy added on top, built for people at risk of falling back into depression. Cognitive therapy is the part of standard talk therapy that works on what you do with a thought once you have caught it. Taught live in a cohort over video, with weekly two-and-a-half-hour classes, a full-day session, and one-on-one time with an instructor. It costs $650.

One program does not sit neatly in either list, because it was built to cover both:

What to Expect

The first surprise is usually that you seem to be getting worse. A few weeks in, you will notice your mind wandering more than when you started. That is not your attention falling apart. Yongey Mingyur Rinpoche calls it the waterfall, and describes hitting it himself: “I was horrified to find myself experiencing more thoughts, feelings, and sensations than I had before I began practicing. It seemed that my mind was becoming more agitated rather than more peaceful.” His teachers told him: “Your mind isn’t getting worse. Actually, what’s happening is that you’re simply becoming more aware of activity that has been going on all the time without your noticing it” (The Joy of Living, 2007).

The first real change shows up outside your practice, not during it. Your formal sessions will probably still feel like a mess. Meanwhile, you will be in the shower, three sentences into replaying an argument from Tuesday, and you will suddenly catch yourself — something that simply did not use to happen. People miss this progress because they are watching their ten minutes for the change instead of watching their day.

The sessions themselves will stay uneven, indefinitely. You will have a great session, followed by one where you cannot stay anchored for two breaths, with no logical explanation you can find. Sleep, hunger, stress, and whatever is unresolved in your life move a single session far more than your technique does. Judging your overall progress by how one sitting went will tell you almost nothing.

The people who start with the worst attention tend to gain the most. When beginners trained for two weeks, the improvement was concentrated in the ones who had been most distracted at the start (Mrazek and colleagues, 2013). If you came to this lesson because your focus is terrible, you are in the group with the most to gain, not the least.

Give this a couple of months and the change starts showing up in how you feel, not just in how your sessions go. Worry takes up less of the day. Your mood holds steadier. Pain, if you live with it, bothers you less. And what you gain, you keep.

Jon Kabat-Zinn built an eight-week program, Mindfulness-Based Stress Reduction, or MBSR, out of the practice of anchoring your attention and bringing it back when you wander. The first people who took his course were followed for years afterward. They came in with anxiety disorders. Three years after the eight weeks ended, researchers reached eighteen of the original twenty-two, and their improvement on anxiety, depression, and panic was still there (Kabat-Zinn and colleagues, 1992). And people who complete MBCT, the version with cognitive therapy built on top, relapse into depression far less often than people getting other treatments (Kuyken and colleagues, 2016).

Attention splits into more than one skill, and each improves on its own schedule. Eight weeks of training was enough for beginners to get better at one specific piece: redirecting attention once it wanders. People who already had years behind them, and who then spent a month on retreat, improved at something else instead: staying alert to whatever was in front of them (Jha, Krompinger and Baime, 2007). Redirecting a wandering mind is a beginner’s gain. Staying alert is a years-long one.

One thing does not change, however far in you go. Your mind still leaves — that is what minds do. Even practitioners with well over a thousand hours behind them lose their place again and again inside a single sitting (Hasenkamp and colleagues, 2012). What changes is how much of the sitting it takes up. People practicing about two hours a day spent roughly a quarter of a long session off in their thoughts. People who knew the same techniques but practiced only occasionally spent closer to sixty percent of it there (Brandmeyer and Delorme, 2018). You are aiming for more stillness, not perfect quiet.

At a scale of practice most people never reach — 10,000-plus hours, the kind measured in decades — something else shows up. Brain regions tied to sustained attention grow more active as practice accumulates, then start to quiet down again in the most experienced practitioners: people with roughly 19,000 hours behind them showed more activation in these regions than beginners, but people with roughly 44,000 hours showed less (Brefczynski-Lewis and colleagues, 2007). That rise and fall is what mastery looks like in any skill. The beginner’s version of a thing is effortful, and it looks effortful in the brain. The expert’s version is the same thing done for less. What is unusual here is how long the road is, and what is waiting at the end of it.

What is waiting is not just a deeper meditation session. It is a fundamental change in the baseline mental experience.

Normally the mind is pulled in many directions. Different parts of the brain run on their own schedules, handling background noise, lingering thoughts, and physical feelings all at the same time. The result is a scattered, distracted experience. But occasionally these separate areas lock into the exact same rhythm, firing together in step across the brain — gamma synchrony, thought to be part of how scattered processing gets bound into a single, unified experience. For the person experiencing it, this pattern is the foundation of a mind operating with calm and focus.

Asked to generate a state of compassion, practitioners with between 10,000 and 50,000 hours behind them produced a rise in that synchrony more than thirty times larger than beginners given the same instruction. The finding that matters most happened before anyone was asked to meditate: at rest, doing nothing, the same synchrony was already elevated in them, and it ran strongest in the people with the most hours behind them (Lutz and colleagues, 2004). They changed their brains through decades of meditation practice, building a highly unified mental state. Clarity was no longer just something they practiced; it was how their minds naturally existed.

That is mastery of focus and attention.

Matthieu Ricard earned a doctorate in molecular genetics at the Pasteur Institute under a Nobel laureate, then left science in his twenties for a monastery in the Himalayas. He has been the Dalai Lama’s French interpreter since 1989, and he was both a subject in the study above and a co-author on it. The press gave him a title he has been trying to hand back ever since: the happiest man in the world. “It’s a big joke,” he told the New York Times. “We cannot know the level of happiness through neuroscience” (New York Times Magazine, 2023).

In another study, researchers hit Ricard with a sudden, unannounced blast of noise to see whether decades of practice could override the human startle reflex. It couldn’t — his body still registered the shock. But when he met it in open, ambient awareness, letting the sound pass through without chasing it or pushing it away, his physical response, heart rate, sweat, muscle tension, came in noticeably lower, and his face barely flinched. Focused, narrow concentration didn’t offer the same protection (Levenson, Ekman and Ricard, 2012).

Ask Ricard how this works, and he strips away the mysticism. The Tibetan word for meditation, gom, translates as “to become familiar with.” He is not claiming to erase the startle reflex, or to permanently shut out the world. Just to familiarize the mind with a more spacious way of meeting it.

When the mind is narrow and self-focused, he says, it takes very little to overwhelm it: “If I put it in this glass, it becomes undrinkable… But if my mind becomes like a big lake, then the same handful of salt doesn’t make much difference; it doesn’t make the whole lake salty” (Taking Charge of Your Wellbeing, University of Minnesota).

Ricard has an image for what it does leave you with: “You can deal with mental perturbations like the eagles I see from the window of my hermitage in the Himalayas deal with crows. The crows often attack them, diving at the eagles from above. But, instead of doing all kinds of acrobatics, the eagle simply retracts one wing at the last moment, lets the diving crow pass, and then extends its wing again. The whole thing requires minimal effort and causes little disturbance” (YES! Magazine, 2009).

That is what is waiting at the end of the road. It is not a mind that never wanders, or a brain that never feels the blast. It is the eagle retracting a single wing. It is the mastery of experiencing the noise of the world without being shattered by it.

The Evidence

Does It Work?

Mindfulness training measurably reduced anxiety, depression, and pain. Goyal and colleagues, 2014 pooled 47 randomized trials and 3,515 people, measured against an active control. Anxiety, depression, and pain all improved within eight weeks, and the gains mostly held three to six months later. The depression effect was almost identical to what antidepressant medication produces against placebo in primary care (Cipriani and colleagues, 2018), and the anxiety effect landed close to what CBT produces once you count the people who don’t finish treatment (Hofmann and Smits, 2008).

The original patients’ gains held for three years. Twenty-two people took part in Kabat-Zinn’s original anxiety study (Kabat-Zinn and colleagues, 1992). Eighteen of them (82%) were reached three years later, and their improvement on anxiety, depression, and panic measures was still statistically significant. Neither this study nor Kabat-Zinn’s chronic pain trial a decade earlier (Kabat-Zinn, 1982) had a comparison group, so they can’t tell you meditation itself was the active ingredient — being met weekly by someone paying close, caring attention to your suffering could be doing some of that work too. What they do establish is that the improvement was real and durable in the people who went through the program.

Mindfulness training works even better in people who are actually diagnosed with anxiety or depression. Hofmann and colleagues, 2010 pooled 39 studies and 1,140 patients with a diagnosed condition and found large improvements in both anxiety and depression. The gains were largest in the patients diagnosed specifically with an anxiety or mood disorder, and were still there at a 12-week follow-up.

Mindfulness training improved daily functioning in veterans with PTSD. Goldberg and colleagues, 2020 pooled 20 studies of nearly 900 veterans with diagnosed PTSD, most of them eight-week MBSR programs. PTSD symptoms improved, and the larger gain was in quality of life and how people managed day to day.

Mindfulness training eased pain regardless of what was causing it. Hilton and colleagues, 2017 pooled 38 randomized trials and 3,536 people with everything from fibromyalgia and back pain to headache, arthritis, and irritable bowel syndrome. Pain eased across the board. Depression improved too.

Mindfulness training works in kids and teens. Dunning and colleagues, 2019 pooled 33 randomized trials of mindfulness programs built for young people — school courses like Learning to BREATHE and the Mindfulness in Schools Programme, adapted from adult MBSR. Looking just at the 17 trials with an active control, depression, anxiety, and stress all fell, and mindfulness itself rose.

Mindfulness training reduced depression and anxiety in adults over 60 too. Ye and colleagues, 2024 pooled five randomized trials of MBCT for late-life depression and anxiety and found moderate effects for both. Hatch and colleagues, 2023 reviewed seven randomized trials of mindfulness programs for older adults — MBSR and MBCT, most of them adapted for the population — and every trial reported less anxiety afterward.

Mindfulness-based programs cut fear of childbirth sharply during pregnancy. Gliske and colleagues, 2026 is a meta-review of 17 systematic reviews on mindfulness during pregnancy and the year after birth. Fear of childbirth was the clearest result — the two reviews that measured it both found large effects, landing on nearly identical numbers. Anxiety improved in every review that measured it (9 of 9), depression in all but one (10 of 11). Stress was the one inconsistent outcome — it improved reliably in people who were already struggling, but showed little room to move in healthy samples who weren’t stressed to begin with.

What Drifting Costs

Almost nothing chronic worriers worry about happens. LaFreniere and Newman, 2020 had twenty-nine people diagnosed with generalized anxiety disorder record their worries for ten days, then followed those worries for thirty. 91.4% did not come true, and the most common individual result was 100%. They were selected for worrying far more than most, so this measures the worry that brings people to treatment.

People who dwell on a low mood are more likely to become depressed later. Nolen-Hoeksema, 2000 found that those who responded to sadness by repeatedly turning it over — replaying what caused it and what it means — were more likely to go on to develop a depressive episode. They were more likely to report anxiety symptoms too.

Minds are elsewhere about half the time, and no kind of wandering beat being present. Killingsworth and Gilbert, 2010 pinged 2,250 adults at random through the day on an app. Minds were elsewhere in 46.9% of the samples. Most of that wandering went somewhere neutral or unpleasant — 31% neutral and 26.5% unpleasant, against 42.5% pleasant. And thinking about something pleasant left people no happier than being absorbed in whatever they were actually doing. Neutral topics left them measurably less happy, and unpleasant topics several times worse than that. Because people were pinged repeatedly over time, the researchers found that wandering tended to come before the dip in mood rather than after it.

People who care more about money and possessions report feeling worse. Dittmar and colleagues, 2014 pooled 259 samples and found the association held, though modestly — strongest for risky spending behavior and harsh self-judgment. It is correlational.

Concrete thinking helps; abstract self-judgment doesn’t. Watkins, 2008 found that what separates the repetitive thinking that helps from the kind that harms is not the subject but how specifically you chew on it. Abstract, evaluative turning-over—why am I like this?—left people recovering from bad moods more slowly than concrete step-by-step thinking, and more slowly than plain distraction. The how question works. The why question mostly doesn’t.

How the Practice Works

There is no anchor that is best for everyone. Anderson and Farb, 2018 had 82 beginners try three different anchors: the breath, a repeated phrase, and a green circle on a screen. Afterwards, 49% preferred the breath, 30% the phrase, and 21% the image. More than half — 56% — ended up preferring something other than what they had predicted before starting.

Wandering, noticing, shifting and holding each run on different machinery. Hasenkamp and colleagues, 2012 had fourteen practitioners press a button whenever they caught their mind wandering in the scanner, which let the four phases be modelled separately. Wandering used the default mode network—the system that runs when people are left alone with their thoughts. Noticing used the regions that flag what’s worth paying attention to. Shifting and holding attention ran on the executive regions, the ones that direct effort.

You miss more than you know. Slagter and colleagues, 2007 sent a stream of letters past people at about twelve a second with two numbers hidden in it. If the second number lands within roughly half a second of the first, most people never see it. The number that went past leaves no trace of having been missed. Seventeen people took that test before and after a three-month retreat of ten- to twelve-hour days, alongside twenty-three beginners tested across the same months. Afterwards all seventeen caught the second number more often without losing the first, against sixteen of the twenty-three beginners improving by less.

Small daily amounts work, and take about two months. Basso and colleagues, 2019 randomly assigned beginners to thirteen minutes of daily meditation or thirteen minutes of a podcast. Mood, attention, and memory improved in the meditation group at eight weeks. None of it had appeared at four weeks. They recruited 76 people and finished with 42, so this rests on the ones who stuck with it.

Practice split into short, cued moments across the day works too. Zainal and Newman, 2023 randomly assigned 110 adults with generalized anxiety disorder to a mindfulness app that prompted them five times a day for two weeks, or a self-monitoring control app with no mindfulness content. The mindfulness group ended up with lower anxiety severity and higher trait mindfulness, both modest effects (d around 0.3 to 0.4).

The key to mindfulness is sustained, long-term practice. Schumer and colleagues, 2018 pooled 65 randomized trials of brief training — anywhere from a single guided session to two weeks of daily practice — and found a small effect (g=0.21) that had faded by the day after training ended, with little sign of a lasting benefit. Correct for the studies with null results that likely went unpublished, and the overall effect drops to g=0.04 — not reliably different from zero. Short sessions are a good way to build the habit. This isn’t a quick fix. It’s a practice you keep.

How often you practice matters more than how long each sitting lasts. Fincham and colleagues, 2023 had 161 people practice daily for two weeks, split between roughly ten-minute and roughly thirty-minute sitting sessions, and found no difference in well-being, distress, or mindfulness between the two groups. Cearns and Clark, 2023 tracked 280,000 real sessions from over 10,000 people and found the same thing at scale: session length barely moved mood scores, but showing up more days a week did — every extra day of practice per week raised mood, and people who practiced four to seven days a week were more than twice as likely to stick with it long-term as people practicing once a week. Bowles and Van Dam, 2025 followed meditators for two to four years and found how often someone practiced mattered roughly two and a half times more than how long their sessions ran.

Home practice is the part that matters. Even across full eight-week MBSR programs, Carmody and Baer, 2009 found more hours of class time didn’t produce a bigger effect. Parsons and colleagues, 2017 pooled 28 studies of standard eight-week MBCT and MBSR courses, where people were assigned about forty-five minutes a day, six days a week, and actually completed about thirty. How close someone came to that target correlated with how well they did.

Short sessions hold up fine early on, and they’re what keeps your mood steady later. In that same 280,000-session dataset, Cearns and Clark, 2023 found that before someone’s twentieth session, short (five to ten minutes) and medium (eleven to twenty minutes) sessions were tied to higher mood than longer ones. After the twentieth session, longer sessions (twenty-one to thirty minutes) moved ahead for mood and for how fast people recovered from a dip. But for keeping mood steady day to day, shorter sessions came out ahead again after about eighty sessions.

Sleep loss hits attention harder than it hits other kinds of thinking. Lim and Dinges, 2010 pooled 70 studies covering 1,533 people and measured six things: simple attention, complex attention, working memory, processing speed, short-term memory, and reasoning. Lapses of simple attention — the moments where you are simply not there — took the biggest hit of the six. Reasoning accuracy barely moved. Sleep loss takes out the specific capacity this lesson is trying to build and largely leaves the rest — not because sleep and practice compete for your time, but because a short night removes exactly what the practice is working on.

What the Road Looks Like

The waterfall is a normal phase for beginners, not a sign anything is going wrong. Harvard Medical School’s Darshan Mehta describes it as a general pattern: heightened awareness that “can initially feel like more stress, but represents greater self-knowledge.” Early on, you’re building a skill and it can feel like you’re losing ground, because the thing you’re building is the very awareness that lets you notice what was always there.

Two weeks of training left undergraduates wandering less — and raised their test scores. Mrazek and colleagues, 2013 randomly assigned undergraduates to two weeks of attention training or a nutrition course. The training group reported fewer off-task thoughts afterward, and scored higher on working memory — how much you can hold in mind and use at once — and on reading comprehension. The gains were biggest in the students who had been drifting the most before they started.

Experienced practitioners wander less than newer ones. Brandmeyer and Delorme, 2018 probed twenty-four practitioners at random intervals during a one-hour sit, and the more experienced half reported wandering less often than the less experienced half. A 2021 review of 24 studies reached the same conclusion.

Side Effects

Most side effects are mild and pass within an hour. Britton and colleagues, 2021 interviewed 96 people who had finished an eight-week mindfulness course, using a 44-item instrument put to them by someone who was not their teacher. 58% reported an unpleasant experience they attributed to the meditation. 37% reported something that reached into life outside the practice. For 57%, the longest such experience lasted under an hour. For 6% to 14% — depending on how long “lasting” has to count, from a day up to five months or ongoing — it did not resolve that fast.

Studies that ask carefully find far more side effects than studies that wait for complaints. Farias and colleagues, 2020 screened 6,742 citations and included 83 studies measuring adverse events. Overall, 8.3% reported one — 3.7% in experimental studies, versus 33.2% in observational ones, where researchers systematically ask instead of waiting for complaints. Britton’s own data shows the same pattern inside a single study, where one open-ended question caught only a third of what the full 44-item interview found.

Talk therapy produces lasting bad effects at a similar rate. In the same Farias review, ordinary psychotherapy produces new or worsening symptoms in 42–51% of people and lasting bad effects in 3–14%. Meditation’s lasting-effect rate falls inside that range.

The rare experiences predicted lasting harm; the common ones didn’t. In Britton and colleagues, 2021, a lasting effect meant a negative impact on functioning, requiring a change in behavior, that persisted at least a day and sometimes months. Four experiences made up most of it — executive dysfunction (trouble planning, organizing, or following through), insomnia, emotional blunting, and a disturbed sense of self — each reported by under 5% of the sample, each raising the risk of a lasting effect 6 to 14 times. More common experiences — anxiety, distortions in time and space, and traumatic memories resurfacing, each reported by 10% to 25% of people — did not predict lasting harm. Two of the risky four, emotional blunting and a disturbed sense of self, were also among the least likely to register as unpleasant while they were happening — so don’t wait to feel bad before treating them as a signal.

Noticing and Allowing

Noticing alone did not move the stress response. Lindsay and colleagues, 2018 gave 153 stressed adults one of three fourteen-day smartphone programs: monitoring plus acceptance, monitoring only, or a coping-skills control. All three were recorded by the same instructor, matched for length and tone, and the word “mindfulness” was never spoken. Faced with a lab stressor, the group trained in monitoring plus acceptance had a lower stress response. Monitoring alone came out level with the coping-skills control.

The Limits

Frequent informal practice carries one small safety flag. In Britton’s study, people who ended up with a bad effect lasting more than a week did not differ in how many minutes of formal sitting they did. They did differ in informal practice — brief bits of attention through the day — at 13.5 sessions a week against 4.3, and informal frequency correlated with the number of events that reached into their lives. The direction is open: people having a hard time may well practice informally more because they are having a hard time. It is not a reason to stop, but it is the nearest thing to a safety signal anyone has measured on frequent brief practice.

The field’s own critics have a long list. Van Dam and colleagues, 2018—a group including researchers sympathetic to the practices—went through what is wrong with this literature. The word “mindfulness” gets used for a dozen different things across studies. Samples are small. Comparison groups often receive nothing at all, which tilts the result. Almost nobody checks for harm. And press reporting runs far ahead of the findings. They were not arguing the practices don’t work; they were arguing the confidence with which they are sold has not been earned.

When a study has no real comparison group, a big effect can’t tell you what caused it. Khoury and colleagues, 2013 pooled 209 studies and found larger effects for anxiety (g=0.89) and depression (g=0.53–0.69) than the tightly controlled trials in Goyal and colleagues, 2014; Grossman and colleagues, 2004, the field’s original meta-analysis, found a similarly large effect (d=0.54) from a base of just seven true randomized trials. Most of the studies behind those bigger numbers were uncontrolled or weakly controlled. None of this means the people in them didn’t improve — many demonstrably did, often durably. It means the studies can’t isolate what did the work: the specific technique, or simply being met each week by a trained teacher paying close attention to your suffering. Goyal’s tighter number holds up because its comparison groups got that same kind of attention too.

Almost all of this research was done on a narrow slice of the world. Foale and colleagues, 2024 screened 112 candidate studies on mindfulness for low-income adults and found only 12 that met inclusion criteria — 10 of them conducted in the US. DeLuca and colleagues, 2018 found the same gap inside Western countries, where ethnoracial minorities are underrepresented in the mindfulness trial literature even where most of the research happens.

Not One Skill

You get the capacity you train, not a general improvement in the mind. The ReSource Project put roughly 330 people through three thirteen-week trainings, rotating the order so each served as the control for the others: one on attention, through breath meditation and body scans; one on compassion, through loving-kindness practice; one on perspective-taking, through noticing a thought as an event and taking another person’s view. Attention improved most after the attention training, compassion most after the compassion training, and perspective-taking least of the three (Trautwein and colleagues, 2020). Cortical thickness changed in a different region for each, and each person’s brain change tracked their own gain on the skill that training worked on (Valk and colleagues, 2017).
